After a player registration has been completed this online medical release form will need to be completed before the player will be allowed to participate in practices or games.

School and Residence verification forms are visible during the player registration process, and unfortunately cannot be hidden due to Little League partner requirements. VAYL is not requiring school and residence verification forms to be completed for any player.
